Your progress and maintenance of your mental well being is dependant on several factors.Two of those factors are taking the correct medication, and having psychotherapy.
These can seem unnecessary at times, but they are just as important to you, as a diabetic taking their insulin.
For this reason, there are special allocations available in one medical aid, so that the available savings will not be consumed by the treatment.
CHRONIC APPLICATION FORMS.
Once diagnosed, you may be prescribed medication that may be obtained from your medical aid from your chronic benefits.
For your convenience we have attached chronic application forms from the more common medical aids. Fill in your sections , sign , and send to Dr Millers rooms drlmillerreception@gmail.com to fill in the script. The practice will send them off on your behalf, to your medical scheme.
Please note there is an administration fee charged to complete the chronic application forms.

The APPLICATION PROCESS
Consult
Consult with Dr Miller to get an assesment.
Diagnosis
Once given a diagnosis, ask Dr Miller , if you can apply for this condition as a PMB, and apply for chronic benafits.
Fill in the application
Download , fill in and sign the application form
Email application
Email the signed application form to the following address: drlmillerreception@gmail.com
Pay the admin fee
There is an admin fee for all forms completed by Dr Miller. This is for the time taken to fill the forms and write a motivation letter to your medical aid.
Follow-up
Wait about a week, then follow up with your medical aid or doctors rooms, if passed.
Book & Attend your psychotherapy sessions
Attend follow-up sessions at regular intervals as directed by the doctor
Reapply every year
Remember , your PMB benefits only last one year. Every January you have to reapply
